In the beginning, they stated there were 3 Patches in the works: around 1:20 The 3rd patch was going to deal with bugs, plus the many user complaints of the PC mouse and keyboard UI. See for more details on the issues being raised. Fixes for these issues are being implemented in the various patches; see the notes below. Note: The hotfix for Patch 3 brought the numbering scheme to 1.04, which their official blog post was updated to show instead of 1.03. The list of fixes is the same.
The patch that went out to XBox and PlayStation was called 'Patch 4' and included both the patch and the hotfix. Patch 5 brought us to version 1.06 - I think Patch 6 brings us to 1.07 but need to confirm.

Later on today we'll be implementing a hotfix for Patch 2 on PC to address graphical issues our players have been experiencing. This fix will ensure that the graphics settings are working correctly for everyone; however, players who are using lower graphics settings will notice changes to the visuals they are used to.
Before Patch 2, released December 9, shader quality for all PC users was being forced to run at Ultra. This impacted the performance of low-end machines and caused some unintended visual artifacts for users whose mesh quality was set lower than Ultra. As a result, users with Low or Medium mesh settings saw hair that was shinier than expected. We attempted to address this in Patch 2 by binding the two settings so that mesh and shader quality were always the same. However, this fix wasn't properly implemented, and it forced shader quality to Low.
We provided a command-line workaround Tuesday, but that still required players to ensure that the new shader quality matched their mesh settings. This hotfix will correctly bind the mesh and shader settings and eliminate this issue. However, now that shader quality is being set correctly for everyone, players using lower settings will notice a decrease in the quality of visuals. This includes the removal of some features such as scars and freckles. Most notably, the lower settings do not support blending of the age layer, so some users may notice that their Inquisitor (or custom Hawke) has aged considerably.
And while this change is how it was originally intended to work, it could be understandably frustrating for people who created their characters before Patch 2. If you would like to correct this change, you can set your mesh quality to High or Ultra. This will enable features that are disabled on lower settings, but may impact your framerate and performance. We're sorry for any stress or confusion this may have caused, and we thank the community for helping us quickly identify and address this issue.
In the upcoming Patch 3 for Dragon Age: Inquisition, our primary focus was on fixing bugs, particularly ones that blocked progression or hampered the player experience. Based on feedback from our players, we tweaked the banter system to be less random in order to prevent extra-long periods of silence. We fixed quest blockers in the personal quests of Dorian and Sera as well as a spawning issue in Solas’s personal quest (I won’t elaborate to avoid light spoilers).
We targeted several issues in multiplayer, fixing the case that caused keys to not drop, as well as fixing issues with the Reaver’s rampage ability. There were also some balance changes, including tweaks to the Demon Commander and the Strength of Spirits ability to make it more useful. We added some much-requested features, including key binding that allows walking for PC and push-to-talk functionality in the multiplayer end of match summary and armor upgrade menus. We also added a new multiplayer Destruction route, and players can now challenge themselves in the fiery Elven Ruins.
You can find a comprehensive list of the upcoming fixes in Patch 3 for PC below. We’ll release more information on patches for consoles in the future, so stay tuned for more updates. Because the development cycle for Patch 3 fell over the holiday period, our priority focus for this patch were items that blocked progress (like key drop or Dorian's quest) and other bug fixes. It's important for us that our devs have time at the holidays to spend with their loved ones and families, and after a long production cycle, this was some well deserved time off. We are still working on future items and improvements for later patches based on the feedback we have received here and elsewhere. I can't give you any specifics as of yet, but as Scylla said, improving PC mouse and keyboard controls is still a priority for us. The fact that we weren't able to get it into patch 3 doesn't change that priority, as not at bugs and features are created (or solved) equal. I can't provide further updates until after Patch 3 is out, but I will give more info once it's possible to do so. On January 26 we released Patch 4 on PC.
It was a small patch focused on two important issues: armor stats for crafted shields and MP Key Drop issue. Previously, crafted shields were using the wrong primary stat – normal armor instead of front armor. This means that in a few scenarios a character would get more armor protection when being hit by the side. On Patch 3 we implemented a change that fixed the armor stat for shields. New crafted shields worked correctly after Patch 3 but shields that were crafted before the fix remained with their old stats which was not displayed properly and not applied to the character when equipped. In Patch 4 we fixed this scenario for old crafted shields.
In Patch 4 we minimized substantially the chance of the MP Key Drop issue happening. We found several instances where monsters could die and not correctly update the internal kill counter—this would cause the end-of-wave key drop event to not occur. We resolved these issues and also added a failsafe mechanism which will attempt to recover in case there are additional cases that have not yet been discovered. We are continuing to investigate other sources of this problem, for example when monsters become unkillable because they manage to escape the playable area. The upcoming Patch 5 for Dragon Age: Inquisition contains a number of improvements to gameplay and stability, as well as some new features. We’ve added party storage and the item tinter to the Undercroft at Skyhold and the ability to adjust the size of subtitles.

PC Specific: While we’ve added the ability to toggle mouse look, this feature is still under development and does not act optimally under all conditions. We are continuing to work on this feature to improve it for future releases. Please note that the Black Emporium is not part of the patch 5 release.
Thanks to the excellent feedback we received from the people in our beta program, we are continuing to work on this feature. Dragon Age™: Inquisition 1.06 Fixed the walk toggle to be framerate independent. Fixed an issue that allowed characters to take damage while using the Counterstrike ability.
Updated abilities to function correctly with front armor rating. Fixed the Charging Bull ability so that AI-controlled characters no longer overshoot their target. Removed the initial barrier that spellbinders receive when spawned. Added an option to control the font size of subtitles. Improved archer AI so that they choose to enter melee combat less often. Fixed the save game card image for elf mages. Removed the connection between mesh quality and shader quality and added a Shader Quality setting to the Graphics options.
Fixed an issue that caused certain operations to disappear from the war table without direct player interaction. Fixed a crash after use of a Tactician’s Renewal amulet in Skyhold. Fixed a conversation where Leliana references being with the Warden to face the Archdemon when she was set to have died in the imported Dragon Age Keep world state. Single-player only: implemented auto-attack with mouse and keyboard controls, with a new key binding created for this purpose. Fixed an exploit where certain abilities could be used multiple times before the cooldown was started. Fixed an exploit with Explosive Shot where indirect hits were not causing the ability to cool down and reduce stamina. Fixed cases of the new-item icon appearing on old items after salvaging in multiplayer. Added an Option to increase the maximum zoom level when using the Tactical Camera.
Patch 6 March 23, 2015 From the. Patch 6 for Dragon Age: Inquisition improves overall game stability, implements a number of bug fixes, and introduces some multiplayer improvements. The Katari multiplayer kit now has more HP and generates guard on hit. Players have been asking for a bigger Multiplayer challenge, so we’ve created Nightmare difficulty, a step up from Perilous in every way. We’ve also introduced a number of new and rare high level weapons, some from the Avvar clans.
Players can now step into the Proving Grounds, a new tutorial level that allows you to test your class, abilities and load out before stepping into the heat of a Multiplayer battle. The upcoming Patch 9 for Dragon Age: Inquisition contains a number of bug fixes and improvements to multiplayer. Patch 9 introduces Pala, the Silent Sister, a new playable multiplayer character.
A member of the Silent Sisters of Orzammar, Pala is a zealous dwarven warrior with an unwavering dedication to the art of war. Pala willingly chose exile to join the Inquisition in its perpetual struggle against the enemies of Thedas. You can now choose from a range of different outfits for your Inquisitor thanks to a wardrobe in Skyhold. Of the Untold Relics of Thedas contest have been selected, and their creations placed in the game. The relics’ locations are a mystery for your Inquisitior to uncover.

Thanks to prominent Thedosian scholars and Shapers, we’ve implemented a series of gameplay adjustments, designed to customize, improve, and balance the player experience. We’ve also added the ability for players to consolidate account bound collectibles across games with a newly added Golden Nug statue located in Skyhold and Haven. With it, you can access your herb seeds, crafting schematics, mounts, Skyhold decorations, and potion recipes across your online saves of the same platform.
Just touch the statue with your post-game character to get access to this feature. The Golden Nug becomes available if one or more of the characters on your account have completed the game at least once. Full details on new features available.
